Job Opportunity: LVN / LPN - Rehabilitation

Location: Utica, New York
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

Position: Licensed Practical Nurse (LPN)
Specialty: Rehabilitation nursing center and Long-Term Care (LTC) experience required
Schedule: Every other weekend
EMR System: PointClickCare preferred
Local radius: 75 miles (local candidates accepted)

Required Qualifications

Valid LPN / LVN license in New York
Minimum 1.5 years of Long Term Care experience, including experience within the last year
Skilled Nursing experience
Experience in a Rehabilitation setting

Additional Requirements

Background checks including 7-year history, EPLS/GSA/SAM, HHS/OIG, education verification, and Florida Education Audit Confirmation (for RN/LPN)
Competency exams as applicable (medication/pharmacology, dysrhythmia/EKG for ICU/PCU/ER staff)
Facility-specific orientation and training modules must be completed prior to first shift
Photo for badge (appropriate headshot)
Various facility-specific documents and acknowledgments must be reviewed and signed prior to start
