Contract Length: 13 weeks

Job Details

• Start Date: ASAP (compliance process takes about 2 weeks)
• Shift: Day shift, 8 hours per day (7:00 AM - 4:00 PM), 5 days a week
• Hours: 36 guaranteed hours weekly (traveler expected to see 5-6 patients per day; guaranteed hours not applied during first 2 weeks, holidays, or time off)
• Patient Ratio: 1:1, may visit up to 6 patients per day
• Work Area: Home Health visits in multiple nearby cities with occasional travel radius up to 50+ miles
• Weekend Work: Optional to meet productivity goals
• On-Call: Not required
• Locals Accepted: Yes
• Attire: Any color scrubs
• Parking: Free
• Mileage Reimbursement: IRS standard

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in a Home Health setting
• Current Basic Life Support (BLS) certification

Job Expectations

• Timely communication and documentation required
• Documentation tools will be provided and must be returned
• System groups clients to minimize drive and windshield time

Compliance & Onboarding Requirements

• Government-issued photo ID (Driver’s License or State ID)
• Emergency Contact Form
• I9 Attestation
• Background checks including 7-year search, SS trace, county searches, nationwide, sex offender registry
• Annual EPLS/GSA/SAM and HHS/OIG checks
• Certifications verification (BLS mandatory; ACLS and PALS may be required depending on contract)
• State license primary source verification
• Medication/Pharmacology competency exams as applicable
• Personal or auto insurance documentation (must be current throughout assignment)
• Auto registration (must be current throughout assignment)
• Physical exam within 1 year prior to start, cleared to work without restrictions
• Immunizations and titers:

  • Tdap within 10 years or annual declination
  • Negative TB test or chest X-ray within 1 year
  • Measles, Mumps, Rubella (2 vaccines or positive titers within required timeframes)
  • Hepatitis B titer or vaccine series or declination
  • Varicella (2 vaccines or positive titer)
• Drug screening: 10-panel drug screen within 1 year prior to start
• Flu vaccine or declination required year-round
• COVID-19 vaccine documentation or declination form required (religious/medical declinations accepted with facility form)
• References: 2 professional references within 3 years for travel/allied/per diem acute care
• Work history verification: 1 year employment verification related to profession
• Annual skills checklist

Additional Notes

• First-time travelers are not accepted for this position
• The facility/system aims to minimize travel time between patient visits
• Compliance and onboarding paperwork must be completed prior to start

This position is ideal for experienced Physical Therapists with home health experience who have a California license and are comfortable managing a full patient load with travel between multiple home locations.
