Physical Therapist - Home Health (Travel/Contract)

Location: Fredericksburg, Virginia (with occasional travel 1-2 days/week to Falls Church area)
Contract Length: 13 weeks
Shift: Day shift, 8:00 AM – 5:00 PM (5 days/week, 8 hours/day, 40 hours/week)

Job Overview

• Manage an average of 4-6 patients per day
• Minimum 24-30 visits per week required to qualify for a 40-hour guarantee
• Patient population: Home Health, adults 18+ years old
• Cases primarily in homecare settings
• Charting must be completed onsite using Kinnser/Wellsky systems; no remote charting allowed
• Free residential parking available
• Dress code: Business casual with closed-toe shoes
• Mileage reimbursed from first to last patient of the day (not to/from home)
• Locals within a 50-mile radius are accepted

Required Qualifications

• Valid Physical Therapist license in Virginia
• Physical Therapist with at least 1 year of experience, including experience within the last year
• Experience in Home Care and Home Health settings
• Basic Life Support (BLS) certification

Compliance & Documentation Requirements

• Government-issued photo ID (Driver’s License or State ID)
• Emergency contact form prior to start
• Background checks within 1 year, including SS trace, county searches (7 years), nationwide, and sex offender registry; updated annually
• EPLS/GSA/SAM and HHS/OIG checks must be done annually
• Certifications: BLS required; ACLS and PALS preferred if applicable
• Physical exam within 1 year of start date
• Negative TB test or chest X-ray within 1 year (with specific protocols for past positives)
• Immunization or titer proof for Measles, Mumps, Rubella, Hepatitis B, Varicella as applicable
• 9-panel drug screen within 1 year of start (instant drug screen acceptable)
• Flu vaccination or declination required during flu season
• Professional references: 2 for travel/allied/per diem acute care; 1 for per diem non-acute care (within 3 years)
• Annual skills checklist and competency exams as applicable

• Punctual and reliable
• Self-starter with strong communication skills
• Efficient and timely with charting (tablet provided for charting)
• Willingness to work and adapt

Additional Notes

• All required documents must be submitted before starting patient care; compliance processing may take 2-3 weeks
• Requests for time off (RTO) must be submitted with application; no additional RTO accepted after offer
