Job Description

Provide physical therapy services in a Home Health setting.
Typical patient diagnoses include Congestive Heart Failure (CHF), Orthopedic conditions, and Pneumonia.
Expected to perform Start of Care (SOC), Recertifications (Recerts), Re-evaluations (Evals), and daily visits.
Work schedule: 5 days per week, 8-hour shifts (8:00 AM to 5:00 PM).
Weekend shifts required on a rotating schedule (8:00 AM to 5:00 PM).
Participate in holiday rotation.
Orientation includes 2 weeks with a preceptor, including a few visits with a work coach.
Scrubs or business casual attire accepted.
Phone and tablet provided for work use.
Clinical manager or educator available 24/7 for emergencies and 8 AM to 5 PM for regular support.

Required Qualifications

Valid Physical Therapist license in Kentucky
Basic Life Support (BLS) certification
