Employment Type: Long-Term Assignment (LTA)

Required Credentials and Certifications

• BLS Certification (American Heart Association or Red Cross)
• Professional License/Certificate Verification (Primary source verification within 30 days of start, updated upon expiration)
• Prime CA Sexual Harassment Prevention Training (Valid for 2 years)
• DCI/BVP Training (Workplace Violence Prevention)
• Driver's License, State ID, or Passport

Required Health and Immunization Documentation

• COVID-19 Vaccine: 2 vaccines plus booster or completed declination
• MMR: Proof of 2 vaccines or positive titer showing immunity
• Hepatitis B: Required (declinations not accepted at some facilities)
• Influenza Vaccine: Required during flu season (Nov 1 - Mar 31) or declination
• TB Test: Within 1 year of start date
• Varicella: Vaccination or titer showing immunity
• Tdap: Required (declinations not accepted at some facilities)
• Physical/Health Statement: Must be completed annually or within 60 days of start, stating clearance to work
• Positive PPD - Negative Chest X-Ray: Optional

Required Testing and Exams

• Prime Core Competencies Exam: Must score 80% or higher
• Prime Skills Checklist
• Prime Specialty Exam: Must score 80% or higher
• Prime Color-blind Test: Annual requirement (Relias accepted)
• Prime Drug Screening: Minimum 10-panel test including THC; for California, requires both 10-panel urine test excluding THC and psychoactive THC saliva test (within 60 days of start)

Background and Compliance

• Prime Criminal Background Check: National check within 1 year of start date (some state-specific exceptions apply)
• Prime SAM/EPLS Search: Must be completed and dated
• Prime Agency Attestation: Completed after all credentials are uploaded
• Prime Form I-9 (Employment Eligibility Verification): Agency attestation accepted
• Prime Job Description: Signed by employee
• Prime Education Verification: Copy of diploma or transcripts accepted if third-party verification unavailable

• Updated Resume reflecting accurate and recent work history
• Two References: One must be from a manager or direct supervisor
