Employment Type: Long-Term Assignment (LTA)
Contract Length: 26 weeks (approximately 6 months)
Shift: Night shift, 12-hour shifts

Job Description

• Must be able to float between units and work weekends.
• Preference given to candidates available for a full schedule through the end of January and the first week of February.
• Position is Per Diem for 26 weeks.
• Minimum compliance requirements must be met prior to starting; full compliance profile must be completed within two weeks after the start date.

Required Certifications & Licenses

• Active nursing license verification (Nursys and primary source verification within 30 days of start)
• BLS (American Heart Association or Red Cross)
• ACLS, PALS, and NRP certifications as applicable
• COVID-19 Vaccine: 2 vaccines and booster required or declination completed
• MMR: Proof of 2 vaccines at least 28 days apart or positive titer
• Varicella: Proof of 2 vaccines at least 28 days apart or positive titer
• Hepatitis B vaccination (declinations not accepted)
• Influenza vaccine or declination required (Nov 1 - Mar 31)
• Tdap vaccination (declinations not accepted)
• TB Test within one year of start date
• Physical/Health Statement completed within 60 days of start, must state clear to work
• Driver’s License, State ID, or Passport

Additional Requirements

• Annual respirator fit test (if applicable)
• National Criminal Background Check within one year of start date
• Prime Core Competencies Exam: Must score 80% or higher
• Prime Skills Checklist
• Prime Specialty Exam: Must score 80% or higher
• Prime Orientation Packet (Essentials Form and Post Test)
• Prime Job Description signed by employee
• Prime Drug Screening (minimum 10-panel test including THC) within 60 days of start
• Employment eligibility verification (Form I-9 or agency attestation)
• Prime SAM/EPLS exclusion check
• Prime Agency Attestation completed after all credentials are uploaded
• Two professional references required (one from a manager or direct supervisor)
• Updated resume reflecting accurate and recent work history
