Job Overview

• Primarily outpatient setting with occasional float to acute care.
• Responsible for patient-focused care including assessment, planning, intervention, and evaluation in accordance with physical therapy standards and physician orders.
• Delegates tasks to Physical Therapist Assistants and Aides.
• Develops and updates individualized treatment plans and goals based on patient assessments.
• Educates patients, families, and caregivers on conditions, treatment rationale, and home programs.
• Provides supervision to assistants, aides, home health aides, and students per regulatory standards.
• Documents patient progress and treatment responses in compliance with regulatory and billing requirements.
• Functions as case manager for assigned cases.
• Participates in interdisciplinary team meetings and promotes physical therapy services within the community.
• Demonstrates effective communication and teamwork skills.
• Maintains confidentiality and adheres to organizational policies and procedures.

Required Qualifications

• Valid Physical Therapist license in Michigan
• Bachelor's degree

Physical & Mental Requirements

• Medium physical demand level:

  • Occasionally lift or carry 21-50 lbs. (0-33% of workday).
  • Frequently lift or carry 20 lbs. (34-66% of workday).
  • Constantly lift or carry 10 lbs. (67-100% of workday).
• Frequent sitting, standing, walking, and postural changes such as stooping, kneeling, or crouching.
• Ability to move freely about the facility with or without assistive devices.
• Overall good vision and hearing with or without assistive devices.
• Ability to handle multiple tasks, maintain regular attendance, and stay alert.
• Dexterity required for handling, reaching, grasping, fingering, and feeling, often repetitively.
• Exposure to stressful situations including trauma, grief, and death.
