Job Opportunity: Physical Therapist (PT) / Speech-Language Pathologist (SLP) - Travel/Contract

Location: San Jose, California (with possible coverage in surrounding counties including Sonoma, Marin, Contra Costa, Alameda, Santa Clara, San Joaquin/Stanilaus, Sacramento)
Specialty: Therapy/Rehabilitation
Employment Type: Travel/Contract
Contract Length: 26 weeks (approximately 6 months)

  • Less experienced clinicians must commit to 6 months (non-negotiable)

Position Details

  • Disciplines Needed:
    • PTs: 1-2 per region
    • SLPs: 1 per region
  • Patient Focus: Home Health, Wound Care, Geriatric Home Health Patients
  • Visits: Minimum 23-24 visits per week required to qualify for a 40-hour guarantee
  • Experience Required:
    • Minimum 1 year in Home Health
    • Minimum 2 years overall as a clinician
  • First-Time Travelers: Accepted
  • Weekend Requirement: No weekends required
  • On-Call: Not required (discretionary)
  • Territory: Will be discussed during interview; may include surrounding counties
  • Locals Accepted: Yes, within a 50-mile radius
  • Mileage Reimbursement: IRS standard (from first patient to last patient, not to/from home)

Licensure & Certifications

  • Must have valid state license in hand at time of submission (no pending licenses accepted)
  • Required Certifications:
    • BLS (Basic Life Support)
    • ACLS and PALS preferred (American Heart Association or American Red Cross)
  • Must comply with all state licensing requirements relevant to the discipline

Work Environment & Policies

  • Dress Code: Any color scrubs, closed-toe shoes
  • Charting System: Kinnser/Wellsky; charting must be completed onsite (no remote/charting from home)
  • Parking: Free residential parking available
  • COVID & Flu Policy:
    • COVID vaccination required (religious declination allowed)
    • Flu vaccination recommended; declination form required if opting out
  • Compliance: All required documents must be submitted and approved before starting patient care (compliance process may take 2-3 weeks)
  • Return to Office (RTO): All RTO requests must be submitted with application; no additions accepted after offer

Required Documentation & Compliance (Examples)

  • Government-issued photo ID (Driver’s License/State ID)
  • Emergency Contact Form
  • Background Checks (7-year search including SS trace, county, nationwide, sex offender)
  • Annual EPLS/GSA/SAM and HHS/OIG checks
  • Physical exam within 6 months signed by MD/DO, including statement of no active communicable diseases
  • Immunizations and titers:
    • Tetanus/Diphtheria/Tdap (within 10 years or exemption)
    • Negative TB test or chest X-ray within 1 year
    • Measles, Mumps, Rubella (proof of immunity or vaccination)
    • Hepatitis B (titer, declination, or vaccine series)
    • Varicella (titer, vaccination, or history)
  • Drug screening: 9-panel within 1 year
  • Seasonal flu vaccination or declination form (during flu season)
  • COVID vaccination or medical/religious exemption
  • Professional references (2 for travel/allied/per diem acute care; 1 for per diem non-acute care)
  • Annual skills checklist and competency exams as applicable
