Overview

Required qualifications

• Valid RN license in California
• Associate of Science in Nursing / Associate Degree in Nursing
• BLS issued by American Heart Association
• Case Management (1 year of experience)
• Case Management experience within the last year
• Experience in a(n) Acute Care setting
• 1-2 years of experience in acute hospital setting as a Case Manager
• Admission criteria knowledge
• Discharge planning
• Disease management
• Plan of care development
• CMS knowledge
• Joint Commission knowledge
• Experience with Epic

Work setting and responsibilities

• Acute care case management
• May involve admission criteria, discharge planning, disease management, needs assessment, and plan of care

Preferred experience

• ER
• Behavioral health
• Workers’ compensation case management
