Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Day shift, 5 days a week (8:00 AM – 4:30 PM)
Hours: 40 hours per week (8 hours per day)

Job Description

• Position focuses on acute care discharge planning or homecare/community discharge planning.
• This is not a counseling MSW position.
• Floating required to nearby facilities within the health system.
• Charting system used: Epic.
• Scrub color: Mocha Brown.
• Weekend requirement: Every 4th weekend.
• Holiday requirement: 1 holiday.
• No on-call duties.

Required Qualifications

• Valid Licensed Master Social Worker (LMSW) license in New Jersey.
• Minimum of 2 years of LMSW experience, including experience within the last year.
• Experience in an acute care setting.
• Experience in a home care setting.
• Experience with homecare/community discharge planning.

Compliance and Background Checks

• 7-year criminal background check within 90 days of start, updated annually.
• Must include all locations lived, worked, or attended school in the past 7 years.
• Federal and state-level checks including EPLS/GSA/SAM and HHS/OIG.
• Education verification or official transcript required.
• Drug screening: 10-panel drug screen within 60 days prior to start.
• Facility-specific modules (~6 hours) must be completed prior to orientation.
• Signed acknowledgment of safety manual and pass/fail testing for dysrhythmia exam during orientation.
• References and skills checklist updated annually.

Health and Immunization Requirements

• Physical exam within 2 months of start.
• Fit mask test (3M 1870+ model) within 12 months prior to start; must be completed before starting.
• Current Tdap vaccine or titer.
• Negative TB test (Qgold or TSPOT) within 90 days of start; chest X-ray if positive.
• Annual TB questionnaire.
• Measles (Rubeola), Mumps, Rubella: 2 vaccines or positive titer; exemptions apply for candidates born before 1957.
• Hepatitis B: Series, titer, or declination accepted.
• Varicella: 2 vaccines or positive titer.
• Seasonal flu vaccine or approved medical/religious exemption required during flu season.
• COVID-19 vaccine no longer required prior to start; however, medical/religious exemptions require mask-wearing at work and COVID booster is required for all New Jersey locations.

Additional Notes

• No on-call responsibilities.
• Floating to other nearby facilities within the health system is required.
• Modules and orientation instructions will be provided directly by the education department.
• Candidates must comply with all facility-specific policies and documentation requirements prior to start.
