Physical Therapist - Outpatient (Travel/Contract)

Location: Coupeville, Washington
Employment Type: Travel/Contract
Shift: Day shift, 5 days a week, 8 hours per day (40 hours/week)
Contract Length: 13 weeks

Job Details

• Schedule: Monday through Friday, no weekends required

Required Qualifications

• Valid Physical Therapist license in Washington
• Minimum 2 years of Physical Therapist experience
• Physical Therapist experience within the last year
• Basic Life Support (BLS) certification required

Required Certifications & Competencies

• BLS (Basic Life Support) certification (AHA only)
• Competency exams as applicable by profession
• Medication/Pharmacology competency exam as applicable

Health & Immunization Requirements

• Physical exam within 1 year of start date
• Fit mask test within 1 year, updated annually (can be completed onsite if necessary)
• Tetanus/Diphtheria/Tdap vaccine within 10 years (declinations accepted)
• Negative TB test (QGold or TSpot) within 12 months; chest X-ray if history of positive TB test
• Annual TB questionnaire regardless of TB test result
• Measles (Rubeola), Mumps, Rubella: titer or completed series; declination form available
• Hepatitis B titer required; if negative, vaccination or declination acceptable
• Varicella titer or completed series
• 9-panel drug screen including marijuana within 1 year of start
• Seasonal flu vaccination or declination form required for new starts (mask required if declined)
• COVID initial vaccine series or declination required

Facility Specific Requirements

• Clear, passport-style photo for badge
• Personal or auto insurance may be required (verify based on specialty)
• Must review and sign unit-specific job description prior to start
• Monthly sanctions search (SAM & OIG) conducted by facility
• Various facility-specific forms required (e.g., immunization contractor form, sanction disclosure, ID badge request)

• No weekend or call requirements
• Floating within scope may be required
• Locals and first-time travelers are eligible to apply
