Job Opportunity: PTA Home Health

Location

• Alameda County, California
• Territory includes cities from Fremont to Berkeley, plus Tri-Valley areas (Livermore, Dublin, Pleasanton)

Contract Length

• 13 weeks

Work Schedule

• Monday to Friday, 5 days per week
• 8-hour shifts, 8:00 AM to 5:30 PM
• Approximately 6 patient visits per day

Required Certifications and Licenses

• California Physical Therapist Assistant (CA PTA) license
• Basic Life Support (BLS) certification (AHA or ARC)

Background and Screening Requirements

• 7-year background check including Social Security trace with all aliases, county searches, nationwide sex offender search, education verification, and motor vehicle report (required for driving positions)
• FACIS Level 3 screening within 2 weeks of start and monthly thereafter
• EPLS/GSA/SAM, HHS/OIG, OFAC sanctions list, Medi-Cal Suspended and Ineligible Provider List checks as applicable
• Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to California law)
• Employment history verification covering 7 years with no gaps over 30 days
• References: 2 professional references within 3 years (Aya requirement, not client)
• Education verification including official transcripts/diplomas if needed

Health and Immunization Requirements

• Physical exam within 1 year of start, cleared for work
• Fit mask test within 1 year, renewed annually (exceptions apply)
• TB screening (negative QGold or TSPOT within 90 days or 2-step PPD) renewed annually
• Immunizations or proof of immunity for:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (series or declination)
  • Varicella
• Seasonal flu vaccination or signed declination during flu season (Nov 1 - Apr 30)
• Drug testing: Negative 9-panel drug screen within 6 months (excluding marijuana/cannabinoids)

Competency and Skills

• Unit-specific skills checklist required, renewed annually
• Medication/pharmacology competency exam as applicable
• Other competency exams (e.g., EKG/Dysrhythmia) may be required depending on unit

Facility-Specific Documents and Training

• Must review and sign facility-required documents prior to start, including:

  • Job description
  • Standards of business conduct
  • Code of conduct
  • Vendor training
  • Overpayment refund policy
  • General regulatory orientation for contracted workers
• Conditional offer of employment attestation required prior to start
• Religious exemption forms required if applicable (for facial hair or vaccine exemptions)

Additional Notes

• Personal or auto insurance required for all candidates due to driving responsibilities
• Photo for badge required for per diem staff
• Prior travel experience is not specified as required for this position

Requirements/Qualifications

• Valid Physical Therapy Assistant license in California
• Minimum of 1 year of Physical Therapy Assistant experience, including experience within the last year
• Basic Life Support (BLS) certification (AHA or ARC)
