Job Opportunity: PTA Home Health (Therapy/Rehabilitation)

Job Description

• Position: Home Health Physical Therapist Assistant (PTA)
• Start Date: ASAP
• Shift: 8-hour day shifts (no 4x10 or 3x12 shifts)
• Patient Ratio: 1:1, may visit up to 6 patients per day (30 productivity points standard)
• Weekend Work: Not required
• On-Call: Not required
• Floating: Not required
• Block Schedule: Not accommodated
• Locals accepted within 50-75 mile radius
• Pending license accepted
• Transportation: Traveler must have own transportation
• Typical daily travel radius: up to 60 miles (60-90 minutes total drive time)

Required Experience

• Minimum 1 year of skilled home health experience
• First-time travelers accepted
• Prior travel experience: Not required

Licensure

• Oregon or Washington state licensure required
• Pending license accepted

Required Certifications

• BCLS (Basic Cardiac Life Support) required

Attire

• Business casual
• Scrubs allowed but not required (no specific color)
• No jeans

Documentation & Compliance Requirements

• Physical exam within 1 year of start, signed by MD, NP, PA, or DO
• Background check within 45 days of start, covering all locations lived/worked in past 7 years
• Drug screen: 10 panel within 90 days of start (no instant tests)
• Immunizations and health documents required:

  • TDAP (within 10 years) or declination
  • Negative TB test or chest X-ray within 12 months
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer
  • Hepatitis B vaccination or signed declination
  • Varicella vaccination or positive titer
  • Flu vaccine during flu season or declination
  • COVID vaccine or declination
• Respirator clearance required annually for travel candidates
• Fit mask testing required annually (onsite if no recent record)

Training & Competency

• EPIC remote client charting system used (laptop and phone app)
• Required to complete Providence Health and Services WorkRamp modules annually, including:

  • Regulatory requirements
  • Specialty-specific modules for home health and therapy/rehabilitation
• Medication and competency exams as applicable
• Other certifications may be required based on clinical track (verify with clinical lead)

Additional Notes

• Facility can call off up to one shift per pay period (every two weeks)
• Free parking and mileage reimbursement at IRS standard rates
• Must comply with Providence Health and Services float policy (may be required to float to other nearby Providence facilities)
• References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within last 3 years
• Employment application must include at least last 5 years of employment with month/year format
