Job Description

• Provide Physical Therapy services in a Home Health setting with home visits and periodic office meetings.
• Average patients per day: 5-6 (assigned 5-10 patients/day equating to roughly 6-8 points).
• Types of cases: Start of Care and Follow Ups.
• Work schedule: Monday to Friday, 8 AM to 5 PM.
• Weekend rotation: No weekend work required.
• On-call: No on-call required.
• Territory: Must be willing to travel up to 75 miles from home clinic, approximately 100+ miles traveled weekly.
• EMR system used: HCHB Point Care.
• Local candidates accepted at the same rate.

Requirements

• Minimum 1 full year of recent Adult Home Health experience required.
• First-time Home Health travelers not accepted; must have recent Home Health experience.
• CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
• Must have reliable transportation and a current driver's license.
• Must have agency-required liability insurance.
• Must have license in hand prior to start (pending licenses accepted only if a few days from issuance).

Additional Information

• Productivity expectations: Approximately 25-30 units per week.
• Must be willing to cover a 50-mile radius around Waxahachie, TX.
• No guaranteed hours; if patient census is low, contractor may be floated to a nearby clinic to pick up hours.

Compliance & Documentation Requirements

• Background checks including 7-year history, sex offender search, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
• Fingerprinting required for certain states and must be current (varies by state).
• Education verification required.
• Drug testing: 10-panel drug screen required.
• Health documents required:

  • Physical exam within 12 months prior to start.
  • Negative TB test or chest X-ray within 12 months, updated annually.
  • Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
  • Flu vaccination or documented declination for current season.
  • COVID vaccination series or declination.
• References: Two references required for all placements.
• Work history verification: Must verify at least two most recent employers or last 7 years if only one employer.

Notes

• Candidates must review and sign various compliance, confidentiality, privacy, and policy documents prior to start and annually if working more than one year consecutively.
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not billable).
• Weekend and on-call requirements may vary by location; discuss during interview.
