Behavioral Health Nurse (LVN / LPN) - Travel/Contract

Location: Worcester, Massachusetts
Contract Length: 13 weeks


Job Details

  • Employment Type: Travel/Contract
  • Shift Options:
    • Day shift, 5 days a week, 8 hours per day (7:00 AM - 3:00 PM)
    • Day shift, 4 days a week, 10 hours per day (7:00 AM - 5:30 PM)
  • Guaranteed 40 hours per week
  • Flexibility required for variable hours and weekend work as needed
  • Must be willing and able to travel within a 35-mile radius to multiple locations with advanced notice

Required Qualifications

  • Valid LPN / LVN license in Massachusetts
  • Minimum 1 year of Behavioral Health experience within the last year
  • Basic Life Support (BLS) certification issued by the American Heart Association
  • Ability and willingness to float to all locations within the 35-mile radius
  • Valid driver’s license or state ID required

Responsibilities

  • Deliver and manage client care using the Nursing Process from admission to discharge, appropriate to client developmental age
  • Perform client assessments and reassessments according to standard procedures
  • Develop and coordinate individualized plans of care, including discharge planning, with the interdisciplinary team
  • Direct, supervise, coordinate, and evaluate nursing care within the clinical setting
  • Delegate tasks appropriately based on job description and competency levels
  • Document client care and education provided to clients/families according to standard procedures
  • Administer medications, perform treatments and therapies, and operate equipment per policy
  • Assist licensed healthcare providers with treatments, procedures, and special tests as directed
  • Participate in performance improvement initiatives and maintain quality and continuity of care
  • Maintain professional ethics, attitude, conduct, and boundaries
  • Stay current with nursing standards and facility policies

Compliance and Documentation Requirements

  • 7-year criminal background check within 30 days of start, including SSN trace, county searches, national criminal search, and other verifications
  • Massachusetts Statewide iCORI clearance within 30 days prior to start
  • Education verification of highest completed education (transcripts acceptable if education verification is unperformable)
  • Drug testing within 30 days of start (specific drug panels required)
  • Physical exam within 12 months of start, including physician statement of fitness to work without restrictions
  • Immunizations or valid declinations for:
    • COVID-19 (most recent vaccine or declination)
    • Flu seasonal vaccination or declination (required during flu season)
    • Tetanus/Diphtheria/Tdap
    • Measles (Rubeola), Mumps, Rubella
    • Hepatitis B (3-dose series, reactive titer, or declination)
    • Varicella (positive titer, vaccination, history, or declination)
  • Tuberculosis screening: Two PPD tests or QuantiFERON/T-Spot within 90 days; chest X-ray if positive TB history, plus TB questionnaire
  • Medication and pharmacology competency exams required prior to start
  • Dysrhythmia/EKG competency exam highly recommended but not required for LPNs
  • Pass/fail medication exam required on first day of orientation with minimum 80% score
  • References: Two references within three years of start
  • Employment application documenting 7 years of employment history with explanations for gaps over 30 days
  • Skills checklist completed prior to start and updated annually

Additional Notes

  • Preference given to candidates with no Return to Office (RTO) restrictions
  • Advanced notice will be provided for daily assignment locations
  • May be required to travel or transport between locations
  • Must maintain compliance with all facility-specific policies and procedures throughout assignment
