Job Opportunity: Physical Therapist - Home Health

Location: Grants Pass, Oregon
Contract Length: 13 weeks


Job Details

  • Specialty: Physical Therapy (Home Health)

  • Patient Population:

    • Primarily geriatrics
    • Adults typically aged 45-65
    • Conditions include post-operative, neurological/CVA, general medical
    • Majority post-acute care with some direct referrals from MDs
  • Types of Visits:

    • Start of Care
    • Evaluations
    • Reassessments
    • Treatments
    • Discharges
    • Recertifications
  • Workload:

    • 25-30 points per week (approximately 2-4 patients per day)
    • 2-4 points per day expected
    • No overtime
    • No on-call duties
