Job Opportunity: RN (Registered Nurse) - LTC

Facility & Department Information

• Facility Type: Skilled Nursing Facility (SNF) and Long-Term Care (LTC)
• Bed Count: Census typically 80-95 residents; 20 rooms are not private
• Department: LTC | RN
• EMR System: PCC
• Nurse-to-Patient Ratios:

  • RN LTC: 1:25
  • RN SNF: 1:13
  • LPN LTC: 1:25
  • LPN SNF: 1:13
  • CNA: 1:10-15 (aim to keep around 10)

Required Certifications & Licenses

• BLS Certification required
• Illinois nursing license required (temp or permanent)
• Driver’s license required for submission

Experience Requirements

• Minimum 1 year of nursing experience required
• Experience with geriatrics preferred
• First-time travelers are welcome (prior travel experience NOT required)

Job Duties & Responsibilities

• Administer IV antibiotics and fluids (no titratable drips)
• Perform patient assessments, medication administration, dressing changes, wound vac management
• Admission and discharge responsibilities handled by RN
• CNAs perform vitals and ADLs (no blood sugars)
• Attend grand rounds/meetings encouraged but not mandatory
• May float between units but generally assigned to the same unit during a stretch

Patient Care & Common Diagnoses

• Common patient conditions: feeding tubes, IV antibiotics, post-surgical care, rehabilitation, long-term care needs
• Respiratory Therapy (RT) available in-house during weekdays; nurses provide treatments evenings/weekends
• RT equipment includes AVAP/CPAP/BIPAP, mostly used at night; some patients on continuous respiratory support

Team & Support

• Interdisciplinary team includes PT/OT/SLP, RT, social workers, MDS nurses, wound care nurse, dietician
• Physician coverage: Hospitalist after hours, Nurse Practitioner during weekdays, some private MDs follow patients
• 24/7 on-call provider available
• Pharmacy on-site with 24/7 availability, including Omnicell and med carts

Scheduling & Shifts

• RN/LPN shifts: 5am-5pm
• CNA shifts: 8 or 12 hours
• Weekend and holiday work: every other weekend/holiday required
• No call or standby requirements
• Block scheduling may be accommodated
• Orientation: One 12-hour shift on the floor; additional support if needed

• Scrub color: Any professional color allowed
• COVID Unit: Patients intermixed, not separate
• Respiratory protection: Both PAPR and N95 masks used
• Local placements accepted if greater than 20 miles from facility

This position is suitable for LTC RNs with at least one year of experience, preferably with geriatrics, who are open to working in a skilled nursing and long-term care environment. Prior travel experience is not required.
