Location: Key West, Florida

Contract Length: 13 weeks
Shift: Days (Saturday through Tuesday), 4 shifts of 10 hours each (core hours 8:00 AM - 4:00 PM)

Job Description
- Provide acute care case management in a hospital setting.
- Independently complete medical necessity reviews for Medicare, Observation patients, and other non-commercial payers.
- Complete required documentation including 3008 forms, PASRR, and referrals to Skilled Nursing Facilities (SNF), Home Health Care (HHC), and Acute Rehabilitation Facilities (ARF).
- Apply knowledge of Medicare conditions of participation, including the 2 midnight rule and 3-day stay requirement for SNF discharges.
- Collaborate with interdisciplinary healthcare teams to manage facility resources, reduce physician practice variations, and improve patient outcomes.
- Focus on decreasing length of stay and monitoring cost-effective patient care across the continuum.
- Determine medical necessity for admissions and continued stays, identify inappropriate or untimely services, assess discharge planning needs for all patient ages, and coordinate delivery of services to meet those needs.
- Facilitate timely and appropriate hospital discharges.
- Identify opportunities for patient care improvement and maintain documentation as required by regulatory agencies.
- Housing is required for the full duration of the assignment at the time of acceptance.
Required Qualifications
- Valid RN license in Florida
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum 1 year of acute care case management experience, with experience within the last year
- Experience in an acute care setting
- Basic Life Support (BLS) certification
