Location: Fort Wayne, Indiana
Employment Type: Staffing - Supplier Sourced
Contract Length: 13 weeks (89 days ≈ 13 weeks)


Job Summary

  • Provide direct patient care to hospice patients and supportive care to their families.
  • Manage an interdisciplinary team including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff.
  • Case manage all assigned patient clinical records.
  • Perform initial and ongoing patient assessments.
  • Develop and implement Hospice Plan of Care in collaboration with physicians and interdisciplinary team.
  • Provide comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
  • Report changes in patient condition to attending and hospice physicians.
  • Maintain accurate and updated medical records and documentation.
  • Practice infection control and standard precautions.
  • Monitor staff scheduling and participate in quality assessment and performance improvement programs.
  • May participate in agency on-call schedule.
  • Maintain knowledge of pain and symptom management in end-of-life care.
  • Conduct supervisory visits with team members monthly.
  • Ensure compliance with policies, procedures, and regulatory requirements.
  • Maintain close contact with patients’ families to provide support and continuity of care.
  • Act as primary liaison between physician, patient/family, and hospice team.
  • Educate patients, families, and caregivers on care needs including medication administration, dressing changes, oxygen use, and hospice philosophy.

Required Skills & Experience

  • Minimum 1 year RN experience in acute care hospital, oncology, long-term care, hospice, or home health (school internships and clinical hours may be considered in some cases).
  • Knowledge of care for terminally ill patients and their families; understanding of hospice care needs.
  • Strong patient assessment skills.
  • Familiarity with roles of all disciplines providing hospice services.
  • Knowledge of community resources.
  • Good oral and written communication skills.
  • Proficient in documentation and program management.
  • Understanding of general nursing practice, managed care principles, and regulatory guidelines (Medicare, Medicaid, ACHC).
  • Ability to operate nursing assessment tools and common office equipment.
  • Must have and maintain reliable transportation for work.
