BLS is required for all direct patient care personnel.
License/Certificate Verification must be dated within 30 days of start.
COVID-19 Vaccine: proof of vaccine, declination, or exemption with agency letterhead; declination not accepted for CA or IL.
MMR: 2 vaccinations with dates or primary source IgG titer with lab print-out.
Fit Test: any mask accepted; model and brand tested must be provided; within one year of start.
Hepatitis B: series of 3 or declination only.
Influenza Vaccine: proof of vaccine, declination, or exemption with agency letterhead; declination not accepted for CA or IL.
2 Step PPD OR IGRA (Quantiferon/Tspot): within one year of start; if positive TB results, chest X-ray must be within 1 year of start with TB questionnaire included.
Varicella: 1 vaccination with date or primary source IgG titer with lab print-out.
Tdap: within past 10 years.
Physical/Health Statement: exam within the last year showing candidate is fit to work.
Trinity 10 Panel Drug Screen: completed within 30 days of start and resulted before start with MRO signature; no THC.
ACLS is required for RNs in CCU, ED, PACU, MICU, and SICU.
