Work Schedule & Details

• 40 hours per week
• Shift: 8:00 PM – 6:30 AM
• Call as needed, approximately 1 day per week
• Systems used: Epic, PACS
• Required Certifications: BLS, ARRT

Required Credentials & Onboarding Requirements

Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel.
• License/Certificate Verification: Must be verified online within 30 days of start date.

Health & Safety

• Fit Test
• Trinity 10 Panel Drug Screen: Completed within 30 days of start, preferred within 14 days. Includes testing for multiple substances (NO THC).
• COVID-19 Vaccine: Proof of September 2024 booster, declination or exemption accepted (except for CA or IL).
• MMR: Proof of 2 vaccinations or IgG titer lab results.
• Hepatitis B: At least first vaccination in series prior to start; titers not accepted.
• Varicella: Proof of 1 vaccination or IgG titer lab results.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• 2 Step PPD OR IGRA (Quantiferon/Tspot)
• Trinity Color Vision Test

Professional & Compliance

• Trinity Code of Conduct Form
• Trinity I9 Form
• Confidentiality & Security Agreement
• Integrity and Audit Services Form
• Motor Vehicle Report (MVR): Dated within 30 days of start.
• Specialty Specific Competency Testing: Must be completed within the last year.
• Conflict of Interest Disclosure Form
• Criminal Background Check: Federal, nationwide, county checks for last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest academic degree or diploma verified.
• EPLS (Excluded Parties List System) Check: Dated within 30 days.
• GSA/OIG Exclusion Check: Dated within 30 days.
• Joint Commission CORE Exam: Within the last year.
• NSO (National Sex Offender) Check: Dated within 30 days, including all aliases.
• System Access Form: Must be completed and uploaded one week before start.
• Specific COVID Questionnaire: Completed and uploaded prior to start.
• Skills Checklist: Provided by agency for clinical contractors.
