Job Opportunity: Respiratory Therapist
Location: Chicago, Illinois
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Work Schedule
- Night shift
- Works every other weekend and every other holiday
- Flexibility with scheduling is a must (no block schedules)
Required Credentials & Certifications for Onboarding
Professional Credentials
- Driver's License, State ID, or Passport
- Prime OIG clearance
- Prime Agency Attestation (completed after all other credentials are uploaded)
- Prime SAM/EPLS verification
- Resume submission required
- Two professional references required
- Social Security Card
- National Criminal Background Check (within one year of start date; some state-specific exceptions apply)
- Badge photo (clear headshot, JPEG format)
- Employment eligibility verification (Form I-9 or agency attestation)
- Signed job description
- Essentials Onboarding Packet
- Prime Orientation Packet (includes "Essentials" form and post-test)
- Onboarding Education Attestation (must be completed prior to clearance to start)
Licenses & Certifications
- Primary Source Verification of professional license/certification (updated within 30 days of start)
- BLS (American Heart Association or Red Cross)
- PALS (American Heart Association or Red Cross)
- ACLS (American Heart Association or Red Cross)
- NRP (American Heart Association, Red Cross, or NALS for NICU, L&D, Postpartum RNs)
- DCI/BVP (Workplace Violence Prevention training; can be completed via Relias or similar platforms)
Personal Health History & Testing
- Prime Drug Screening (minimum 10-panel test including THC; within 60 days of start)
- Prime Color-blind test (annual; Relias accepted)
- COVID-19 Vaccine (2 vaccines plus booster or completed declination)
- MMR (proof of 2 vaccines or positive titer)
- Annual Respirator Fit Test
- Hepatitis B vaccination (declinations not accepted at some locations)
- Influenza Vaccine or declination (required October 1 - May 31; declinations not accepted at some locations)
- TB Test and Annual TB Questionnaire
- Varicella vaccination or immunity proof
- Tdap vaccination (declinations not accepted at some locations)
- Physical/Health Statement (annual; must state clear to work)
Testing & Competency
- Prime Core Competencies Exam (must score 80% or higher)
- Prime Skills Checklist (submission required)
