Travel RN LTAC ICU Position
Location: Hazard, Kentucky
Specialty: RN LTAC ICU

Contract Length: 12 weeks
Job Requirements
- Minimum 2+ years current LTACH experience
- Must have Kentucky or compact nursing license
- BLS, ACLS, NIH certifications required
- Experience with 1:4 nurse-to-patient ratios
- Familiarity with Meditech EMR system
- RN must reside within 100 miles of the facility to be considered
- Orientation occurs bi-weekly
- Work schedule: 36/48 hours over a two-week pay period
Required Certifications & Licenses
- BLS (American Heart Association, front and back of certificate if not electronic)
- ACLS (Required for all RNs except Psych and True Med/Surg units)
- NIH Certification (Required for RN specialties except OR and Psych)
- Nursys License Verification (Within 30 days of start)
- Kentucky or Compact License (Updated on extensions, annual update required)
Health & Immunization Requirements
- COVID-19 Vaccine: Proof of completed series or approved religious/medical exemption with documentation
- MMR: Proof of 2 vaccinations or positive titer prior to start; booster or series initiation allowed with timing requirements
- Hepatitis B: Positive titer, 3 vaccine series, or declination accepted
- Varicella: Proof of 2 vaccinations or positive titer prior to start; booster or series initiation allowed with timing requirements
- Tdap: Valid within 10 years; declinations accepted
- Influenza Vaccine: Required October-March; exemptions require approval and must be applied for within 48 hours of offer acceptance
- TB Quantiferon Test: Within one year prior to start
- Annual Physical/Health Statement: Signed by MD, DO, PA, or NP within one year prior to start, stating clear to work without restrictions
Background & Compliance
- Background check within 30 days prior to start including 7-year county, nationwide criminal, SSN trace, sex offender, SAM, OIG checks
- Drug screen within 30 days prior to start (excludes marijuana/THC)
- ARH Kentucky Vulnerable Adult Maltreatment Registry verification within 30 days prior to start
- Submission packet required including cover sheet, resume, skills checklist, driver’s license, and license/certification verification in one PDF
- Assignment confirmation form must be uploaded within 24 hours of offer acceptance
Skills & Competency
- Unit-specific skills checklist within one year prior to start
- Competency exam within one year prior to start (actual documents required, no certificates)
- EKG and Telemetry exam within one year prior to start (actual documents required)
- Core competencies including HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives (annual update required)
References
- Two professional references required: one charge nurse or higher, one supervisor reference within the last year
