Registered Nurse – Step Down Unit

Location: Hazard, Kentucky

Shift Details

• Night shift options:

  • 3 shifts per week, 12 hours each (7:00 PM – 7:30 AM)
  • 4 shifts per week, 12 hours each (7:00 PM – 7:30 AM)
• Total hours per week: 36

Required Qualifications

• Valid RN license in Kentucky
• Associate of Science in Nursing / Associate Degree in Nursing
• Minimum 1 year of Progressive Care Unit (PCU) experience within the last year
• Advanced Cardiac Life Support (ACLS)
• Basic Life Support (BLS) issued by American Heart Association
• NIH Stroke Scale (NIHSS) certification
• Orientation Safety Manual Acknowledgement

Experience

Required Documentation & Compliance

• Unexpired government-issued photo ID (Driver’s License or State ID)
• Background checks including:

  • FACIS 3 or equivalent, SAM (EPLS), OIG, OMIG, OFAC
  • Education verification
• Employment application/resume with education and work history
• 7-year work history verification (third-party vendor or attestation letters)
• Two professional references within 1 year of start date (one must be a supervisor)
• Signed and dated specialty job description (renewed annually)
• Annual skills checklist

Health & Immunization Requirements

• Annual physical exam or physician note within 1 year prior to start, stating clearance to work without restrictions
• Fit mask test to be completed onsite (PAPR not accommodated if facial hair present)
• Tetanus/Diphtheria/Tdap vaccine valid within 10 years (Td not accepted; declinations accepted if within 1 year of start)
• Negative TB test via QuantiFERON blood test within 1 year prior to start (T-Spot/PPD not accepted)

  • If positive TB, chest X-ray within last 5 years plus TB questionnaire required
• Proof of immunity or vaccination for:

  • Measles (Rubeola) – 2 MMR vaccinations or positive titer
  • Mumps – 2 MMR vaccinations or positive titer
  • Rubella – 2 MMR vaccinations or positive titer
  • Varicella – 2 vaccinations or positive titer
  • Hepatitis B series, titer, or declination (declination must be within 1 year of start)
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (9/1–3/31)
• Drug testing within 30 days of start using specified 9-panel bundle (no THC) with expanded opiates and oxycodone

Facility-Specific Requirements

• Completion of Employee Health Attestation & Instructions Form prior to compliance deadline
• Upload proof of TB results, TB risk assessment, OSHA respirator medical evaluation, and flu vaccine/exemption documentation (screenshots with candidate’s name required)
• Review and acknowledge Fit Test Policy
• Completion of Appalachian Regional Healthcare-specific forms for medical or religious exemptions if applicable
