Location: Concord, California
Contract Length: 13 weeks

Specialty: Therapy/Rehabilitation (Home Health)
Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM)
Average caseload: 5-7 patients per day
Patient population: Geriatric
Territory: Up to a 60-mile radius from the hub/office (Far East area: Discovery Bay to Pittsburg)
Guaranteed 40 hours per week (must meet productivity points)
No weekends or on-call required
Dress code: Scrubs (any color), no jeans
Mileage reimbursed at IRS standard rate
iPad provided for work; cell phone use not allowed

Government-issued photo ID (Driver’s License or State ID)
Background checks within 30 days prior to start, including federal, county, motor vehicle, and sex offender registry searches
Annual EPLS/GSA/SAM and HHS/OIG checks
Education verification (diplomas/transcripts or third-party verification)
Physical exam within 6 months prior to start
Negative TB test (PPD, Quantiferon Gold, or T-spot) within 6 months of start; chest x-ray acceptable if history of positive TB
Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (vaccine series or declination)
- Varicella (titer, vaccine, or history of chickenpox)
Seasonal flu vaccination or documented declination during flu season (Oct 1 - Mar 31)
Drug screening: 10-panel drug test within 30 days of start (instant tests acceptable)

Verification of last 3 employers required
Two professional references required for travel/allied/per diem acute care roles (within 3 years)
Annual skills checklist and competency exams as applicable
