Required Qualifications

• Valid Radiology Technologist license in Indiana
• Radiology Technologist experience within the last year
• 1 year of Radiology Technologist experience
• ACLS issued by American Heart Association or American Red Cross
• BLS issued by American Heart Association or American Red Cross
• PALS issued by American Heart Association or American Red Cross
• State license verification required
• Tech certification verification required
• Orientation Safety Manual acknowledgement
• 7-year background check required within 30 days of start, including State Medicaid, SSN verification, Education Verification, and National Sex Offender Registry
• EPLS/GSA/SAM required within 30 days of start
• HHS/OIG required within 30 days of start
• Education verification required
• NURSYS verification required for RN, LPN/LVN, and AP
• Medication Test/Pharmacology Competency Exam as required by profession
• Competency Exam as required by profession/specialty
• Clear passport-style photo for badge
• Attestation required within 48 hours of candidate signing
• Parking permit application required for candidates parking onsite
• Physical exam required within 30 days of start
• Negative TB or Chest X-Ray required within 1 year of start
• Rubeola: positive titer or 2 vaccines
• Mumps: positive titer or 2 vaccines
• Rubella: positive titer or 2 vaccines
• Hepatitis B: positive titer or series of 3
• Varicella: positive titer or 2 vaccines
• 9-panel drug screen required within 30 days of start
• Flu vaccination or religious/medical exemption required during flu season
• RespSafety clearance required
• 2 references within 3 years of start

• Candidate does not need to be compliant for the attestation form to be completed
• N95 fit testing occurs during orientation
• Internal evaluation occurs 90 days after start
