Overview

• Outpatient clinic assignment in Lacey, WA.
• Primarily based in Lacey, with floating 3-4 times per month to Aberdeen and Centralia as needed for coverage.
• Mileage will not be reimbursed.
• Interview required before offer.
• ASAP start; 10/05 preferred, 10/12 acceptable.
• True travelers or locals are accepted.
• Day shift: 4x10 hours, 7:00-17:30.

Required Qualifications

• Valid LPN / LVN license in Washington.
• 2 years of clinic experience.
• Clinic experience within the last year.
• NURSYS verification, as applicable.
• BLS.
• State license verification.
• Education verification.
• Background check.
• Washington State Patrol search.
• Employment application with at least the last 5 years of employment.
• References: minimum of 2 references covering 1 year of employment total within the last 3 years in a comparable specialty/setting.
• Skills checklist.
• Medication test/pharmacology competency exam, as applicable by profession.
• Competency exam, as applicable by profession/specialty.
• Physical exam within 1 year of start.
• Fit mask.
• Tetanus/Diphtheria/Tdap within 10 years.
• Negative TB or chest X-ray.
• Rubeola (Measles).
• Mumps.
• Rubella.
• Hepatitis B.
• Varicella.
• Drug testing.
• Flu seasonal vaccination/documentation.
• COVID documentation.
• RespSafety clearance.
• WA - Multi State Licensed Nurses - Demographic Attestation, if using an out-of-state compact license in Washington.
• WA - Multi State Licensed Nurses - Attestation (APN only), if applicable.
