Job Opportunity: Registered Nurse (Home Health)

Location: Alameda, California

Job Summary

Under the supervision of the Clinical Manager, the Team Nurse delivers direct patient care services as assigned by the Primary Care Clinician and approved by the Clinical Manager. Responsibilities include:

• Implementing physician plans of care and obtaining physician orders
• Supervising home health aides
• Coordinating care within an interdisciplinary team
• Participating in utilization management and control of patient supplies
• Providing age-appropriate care considering developmental, psychosocial, environmental, educational, and communication needs
• Ensuring care follows professional standards and agency protocols

Work Schedule: On call as needed; no floating or weekend shifts

Required Certifications

• California Registered Nurse (CA RN) license
• AHA Basic Life Support (BLS) certification
• Advanced Cardiac Life Support (ACLS) (AHA or ARC) – verify by position
• Pediatric Advanced Life Support (PALS) (AHA or ARC) – verify by position
• Neonatal Resuscitation Program (NRP) – AHA/AAP only, must be Instructor or Advanced Provider Course (verify by position)
• Other certifications may be required depending on position (e.g., CCRN, TNCC, EFM, CPI)

Experience Requirements

• Specialty: Registered Nurse
• Years of Experience: Not explicitly stated; assume minimum 2 years in specialty (default to higher if conflicting info found)

Background and Screening Requirements

• 7-year background check including SSN trace, county searches, sex offender search, education verification, and motor vehicle report (for driving positions)
• FACIS Level 3 screening within 2 weeks of start and monthly thereafter (or equivalent checks if FACIS III cannot be run)
• EPLS/GSA/SAM and HHS/OIG checks, updated annually
• Education verification (Bachelor of Science in Nursing preferred)
• Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law)
• References: 2 professional references required (Aya requirement, not client)
• Employment history: Full 7 years of work history with no gaps over 30 days; verification required

Health and Immunization Requirements

• Physical exam within 1 year of start (must indicate cleared for work)
• Fit mask test within 1 year, renewed annually (exceptions apply)
• Immunizations and/or titers required:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Negative TB test or chest x-ray within 90 days, renewed annually
  • Measles (Rubeola), Mumps, Rubella (MMR) immunity or vaccination
  • Hepatitis B series or declination
  • Varicella immunity or vaccination
  • Seasonal flu vaccination during flu season (11/1-4/30) or signed declination
• Drug testing: Negative 9-panel drug screen within 6 months of start
• Color blind exam if applicable (in-person exam required)
• Respiratory safety clearance prior to start, renewed annually (exceptions apply)
