Job Opportunity: Social Worker (Travel/Contract)

Location: Gurnee, Illinois
Contract Length: 12 weeks (approximately 3 months)
Shift: Monday to Friday, 8-hour days with a 30-minute unpaid lunch (37.5 billable hours per week)

Job Description

• Covering leave at an Intermediate School in Gurnee, IL
• Must hold a valid Illinois Professional Educator License (IL PEL)

Required Experience

• Specialty: Social Services (Social Worker)
• Years of Experience: Not explicitly stated; assume minimum licensure and relevant experience as per IL PEL requirements

Certifications & Licenses

• Illinois Professional Educator License (PEL) required

Background Checks & Compliance

• Criminal background check within 1 year of start, updated annually
• EPLS/GSA/SAM and HHS/OIG checks required within 1 year of start, updated annually
• Illinois VECHS Fingerprint Results required prior to start; fingerprinting handled by the agency
• Illinois Sex Offender Registry and State Police Murderer and Violent Offender Against Youth Registry searches required
• Mandatory Reporter Training (Illinois Mandated Reporter Training) required and must be updated annually
• Educational Services - State Teaching Certificate and FERPA compliance required and updated annually
• Faith’s Law forms (Sexual Misconduct Disclosure and Authorization for Release) required prior to start

Health & Safety Requirements

• Physical exam required within 90 days of start (Illinois state mandate)
• Negative TB test or chest X-ray within 90 days of start (Illinois state mandate)
• TB Questionnaire must be completed annually
• Drug testing not required unless specifically requested by client
• Flu vaccination not required unless requested by client

Additional Notes

• Orientation Safety Manual Acknowledgement required annually
• Competency exams generally not required unless specified by client
• Various accounting and tax documents required (W4, I9, Direct Deposit Authorization, etc.)
• Emergency Contact Form and Equal Opportunity Employment (EOE) forms required
