Position Details

  • Pass/Fail Testing (EKG/Medication): Required for awareness on multiple units; candidates have 3 attempts to pass with remediation required after failures

Background & Health Requirements

  • Background Check: Required within 6 months of start; must include SSN trace, 7-year county residence, nationwide search, sex offender search, and international search if applicable
  • Physical Exam: Required within 1 year, signed by physician, NP, or PA-C; must be updated annually
  • Drug Testing: Negative 10-panel drug screen required within 30 days prior to start
  • Immunizations & Titers:
    • Tetanus/Diphtheria/Tdap (within last 10 years; no declinations accepted for new hires)
    • Measles (Rubeola) - titer or 2 vaccines required
    • Mumps - titer or 2 vaccines required
    • Rubella - titer or 2 vaccines required
    • Varicella - titer or 2 vaccines required
    • Hepatitis B - vaccine series, positive titer, or signed declination accepted
  • TB Screening: Negative TB test or chest X-ray within 1 year of start; annual renewal required
  • Flu Vaccination: Required year-round; medical or religious exemptions accepted with facility-specific forms; must be completed by November 10, 2025, for 2025-26 season
  • COVID-19 Vaccination: Not required but accepted if available
