Job Description
- Position: Licensed Practical Nurse (LPN)
- Required Experience: Minimum 1.5 years
- Required License: New York LPN license
- Experience in Rehabilitation Nursing Center and Long-Term Care (LTC) preferred
- Schedule: Every other weekend
- EMR System: PointClickCare preferred
- Local candidates within 75 miles radius considered
Required Certifications
- ACLS (Advanced Cardiac Life Support) – AHA strongly preferred
- BLS (Basic Life Support) – AHA strongly preferred
- PALS (Pediatric Advanced Life Support) – AHA strongly preferred
- TNCC (Trauma Nursing Core Course) – Required for ER staff
- State License verification (primary source online verification)
- NIH Stroke Certification – Required for ICU, PCU, ER, CT ICU, and certain med/surg units
Required Competencies
- Medication Test/Pharmacology Competency Exam (minimum 80% score or meet/exceed Angoff score)
- Competency Exam (minimum 80% score or meet/exceed Angoff score)
- Dysrhythmia/EKG Competency Exam – Required for ICU, PCU, ER, CT ICU, and certain med/surg units; expires every 3 years
Required Documents & Compliance
- Government issued photo ID (Driver’s License or State ID)
- Emergency Contact Form (required prior to start)
- Background Checks:
- Standard 7-year search including SS Trace, County searches, Nationwide, Sex Offender
- EPLS/GSA/SAM (annual)
- HHS/OIG (annual)
- Education Verification
- Florida Education Audit Confirmation (for RN/LPN staff)
- Physical Exam within 1 year of start
- Fit Mask test within 1 year of start, updated annually (specific N95 masks required)
- Negative TB test or Chest X-Ray within 1 year of start (specific requirements for PPD, Quantiferon Gold accepted, Tspot not accepted)
- Immunizations or proof of immunity:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or documented history)
- Drug Testing: 9 Panel Drug Screen within 1 year of start (instant drug screen acceptable)
- Seasonal Flu Vaccination or Declination (required during flu season: Sept 1 - March 31)
- Pneumonia Vaccination or Declination (required for all candidates)

- Completion of online learning modules prior to first shift
- Review and sign facility-specific documents including:

- Code Elopement Handout
- New Employee Orientation Booklet
- Welcome Packet acknowledgment

- Omnicell Form (for nursing staff)

- Paper orientation documents must be completed prior to start

- 2 Professional References required for Travel, Allied, Per Diem Acute Care
- 1 Professional Reference required for Per Diem Non-Acute Care
- Skills Checklist required annually
