Location: Great Falls, Montana

Health & Immunization Requirements

- Negative TB test (PPD, Q-Gold, or T-spot within 30 days) or chest X-Ray if history of positive TB
- Measles (Rubeola): 2 MMR vaccines or positive antibody titer
- Mumps: 2 MMR vaccines or positive antibody titer
- Rubella: 2 MMR vaccines or positive antibody titer
- Hepatitis B: Titer, vaccine series, or declination
- Varicella: History of chickenpox, 2 vaccines, positive antibody titer, or declination (with work restrictions if exposed)
- Drug Testing: 5, 7, or 10 panel drug screen within 1 year including THC, Cocaine, Amphetamines, PCP, Opiates
- Flu Vaccine: Not required by client, but required by agency standards
- COVID Vaccine: Highly recommended but not required; if not vaccinated or missing booster, must complete declination form
