Job Opportunity: LVN / LPN - Home Health (Travel/Contract)

Location: Largo, Florida
Contract Length: 13 weeks

Job Description

• Provide skilled home health nursing care (Medicare, Medicare Advantage, and Commercial insurance patients)
• Perform 6-8 home skilled visits per day
• Day/shift hours vary based on HomeCare office needs (examples include Mon-Fri 8a-5p, 9a-6p, Thurs-Mon or Fri-Tues shifts, alternating weekends)
• Visits and hours are self-scheduled based on office needs

Patient Care Responsibilities

• Infusion experience required
• Skilled home care experience required (private duty nursing experience not accepted)
• Home health experience with OASIS training and experience
• IV skills and wound care
• General medical diagnoses including high-risk diagnoses such as:

  • Heart Failure (HF)
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Myocardial Infarction (MI)
  • Pneumonia
  • Stroke
• Post-operative care
• Cardiopulmonary assessment
• Basic wound care, ABI compression dressing, wound vac dressing
• Pleurx/Aseptdrain care
• Foley catheter change and care
• Staple/suture removal
• Use of glucometer and pro-time machine
• SQ/IM injections
• Lab draw venipuncture
• PICC line or Midline care and dressing change
• SQ port access/deaccess

Required Qualifications

• Valid LPN / LVN license in Florida
• Minimum 1 year of home health experience within the last year
• Experience working with high-risk patients
• Experience in a home care and home health setting
• Infusion experience
• IV skills and wound care
• Experience with OASIS
• Basic Life Support (BLS) certification issued by the American Heart Association

Documentation and Compliance Requirements

• Driver’s License must remain active and current throughout assignment
• Background checks including:

  • Standard 7-year history check
  • EPLS/GSA/SAM
  • HHS/OIG
• Foreign equivalency evaluation if education completed outside USA or Puerto Rico
• Education verification (transcript or diploma) required
• Florida AHCA Level II fingerprint clearance required prior to start (Livescan electronic fingerprinting)
• Physical exam within 6 months of start date
• Negative TB test or chest X-ray within 10 weeks of start (or chest X-ray and TB questionnaire if prior positive PPD)
• Annual TB questionnaire required in lieu of annual TB test
• Immunizations or titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (titer, series, or declination)
  • Varicella
• Standard 9-panel drug test within 30 days of start (instant tests not accepted)
• Seasonal flu vaccination or declination with mask requirement (deadline for pre-start travelers: 10/7/2024)
• COVID-19 vaccination or approved exemption required
• Respirator medical clearance annually if respirator use is required
• Two professional references from the past 2 years
• Employment application and skills checklist required
• Competency exams as applicable, including:

  • Medication/Pharmacology competency exam for LPN/LVN in Long Term Care
  • Color Blind Testing (Relias) prior to start
• BayCare Health MSP specific requirements including:

  • Dress code and tobacco policy acknowledgment
  • Account registration and completion of mandatory online modules prior to start (approx. 4-8 hours)
  • Photo for badge (clear, professional, solid background)
  • Personal or auto insurance coverage for Home Health positions
  • BayCare Health MSP AHCA privacy policy, affidavit, and demographic information sheet

Additional Notes

• Scrubs: Any solid color scrub
• Laptop, MiFi, and portable printers provided for documentation and scheduling
• Documentation systems used: BayCare Forms, BayCare Scheduling, Ace Timesheet
• Candidates must complete all required BayCare online training modules before starting assignment
• Mask wearing is mandatory if flu vaccine is declined
• Visits and hours are flexible and self-scheduled based on office needs
