Job Opportunity: Case Manager - Registered Nurse

Location: Leominster, Massachusetts
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Overview

• Primarily responsible for discharge planning including initial assessments and follow-up with the same patients.
• Minimal Utilization Review involvement as a dedicated team handles this.
• Patient ratio varies between 15-25 patients per day.
• Charting system used: Epic.
• Work hours are somewhat flexible (e.g., 7 AM - 3 PM or 9 AM - 5 PM).
• Weekend requirement: Every third weekend.
• Previous community hospital experience preferred.
• Prior travel experience not required.

Required Qualifications

• Valid Registered Nurse (RN) license in Massachusetts
• Minimum 2 years of case management experience, including experience within the last year
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Additional Requirements & Compliance

• Valid government-issued ID required.
• Background checks including 7-year criminal history and education verification.
• Health documents including physical exam within 12 months, immunizations or declinations for TB, MMR, Hepatitis B, Varicella, Tdap, Flu, and COVID vaccines as per facility policy.
• Drug testing required within 30 days of start.
• Pass/fail medication and telemetry exams may be required depending on unit assignment.
• References: 2 references within the last 3 years.
• Employment application must cover 7 years of employment history with explanations for gaps over 30 days.
• Skills checklist required prior to start and updated annually.
