Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Travel/Contract

Location: Stormville, New York
Contract Length: 26 weeks

Job Description

• Provide nursing care under the direction of an RN, physician, or other authorized healthcare provider.
• Administer medications as directed.
• Provide bedside nursing care, including sterile techniques.
• Observe, measure, record, and report patient health status.
• Perform specialized tasks routinely such as catheterizations and suctioning; perform IV therapy sparingly with additional training.
• Administer blood and blood products.

Required Qualifications

• Valid New York State LVN/LPN license.
• Minimum of 1 year of work experience post-licensure.
• Current certifications in:

  • ACLS (Advanced Cardiac Life Support) – American Heart Association or American Red Cross only
  • BLS (Basic Life Support) – American Heart Association or American Red Cross only
  • PALS (Pediatric Advanced Life Support) – American Heart Association or American Red Cross only
• Successful completion of competency exams as required.
• Must pass a 9-panel drug test within 1 year of start date.
• Physical exam within 1 year of start date.
• Negative TB test or chest X-ray (Qgold, Tspot, or 2-step PPD) within 12 months prior to start.
• Annual TB questionnaire completion.
• Vaccination documentation or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (or declination)
  • Varicella
• Seasonal flu vaccination or declination for 2025-26.
• COVID-19 vaccine booster preferred but not required.
• Criminal background check fingerprinting required (to be completed onsite).
• Background checks including SS Trace, County searches, Nationwide, and Sex Offender registry within 1 year of start for the last 7 years of locations lived/worked/schooled.
• Two professional references required.

Additional Information

• This is a travel/contract position with a state-run facility; the hiring process may be slow due to bid and review procedures.
• Fingerprinting and background checks are mandatory and must be completed as part of onboarding.
• Medicaid enrollment documentation may be required depending on assignment specifics.
