Job Details

• Shift: Day shift, 4 days per week, 10 hours per day (7:00 AM – 5:30 PM)
• Total hours per week: 40
• On-call availability required: 90 hours of cardiovascular specialty call
• Weekend work required
• Meal break: 30 minutes

Required Qualifications

• Valid Registered Nurse (RN) license in Illinois
• Bachelor of Science in Nursing (BSN)
• Minimum of 1 year of CVOR experience, with CVOR experience within the last year
• American Heart Association Basic Life Support (BLS) certification

Compliance and Documentation Requirements

• Complete and maintain I9 documentation
• Current and valid Driver’s License or Government Photo ID recommended
• Proof of name change if legal name differs from compliance documents
• 7-year criminal background check within 30 days of start, including SSN trace, aliases, county, national, federal, sex offender, OIG, SAM, FACIS 3 checks
• EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
• Education verification of highest degree (BSN minimum)
• State RN license primary source verification within 30 days of start
• NURSYS verification for RN license within 30 days of start
• Passing competency exam required
• Physical exam within 1 year of start confirming good health and no physical limitations
• Fit mask test (3M Aura 1870+) within 12 months of start and annually thereafter
• Drug screening (10 panel) within 30 days of start
• Immunizations and health screenings:

  • Tdap vaccine within last 10 years (TD not accepted)
  • Negative TB test or chest x-ray as applicable
  • Measles, Mumps, Rubella, Varicella immunity or vaccination series
  • Hepatitis B vaccine series or positive titer (declinations accepted)
  • Seasonal flu vaccination or approved medical/religious exemption during flu season
  • COVID-19 vaccination series or declination form
• OSHA Respiratory Safety clearance if fit test failed
• Illinois Healthcare Worker Registry fingerprinting and eligibility verification required prior to start (must be completed in Illinois)

Additional Requirements

• Completion of facility-specific training modules and agreements prior to start, including:

  • Confidentiality Agreement
  • Information Security Agreement
  • Privacy Policy Acknowledgement
  • Sexual Harassment Training (annual update required)
  • Cannabis Acknowledgement
  • Scanning Technology Consent Release
  • Policy Acknowledgement form (effective 3/10/2025)
• Accommodation request forms for vaccine or fit test exemptions must be submitted within 24-48 hours of contract signing for review
• Employment history covering 7 years with explanations for gaps over 30 days
• Skills checklist updated annually

Summary

This is a 12-week travel contract position for a CVOR Registered Nurse in Illinois requiring a BSN, Illinois RN license, BLS and ACLS certifications, and compliance with extensive background, health, and immunization requirements.
