Schedule

• Monday - Thursday
• 2 days: 7:00 AM - 5:30 PM
• 2 days: 8:00 AM - 6:30 PM

Job Description

• Position in a Pre-Screen Unit; clinical fit will be determined through clinical screening and manager interview.
• May involve caring for COVID-19 patients.
• Floating across county lines possible if patient care requires.
• Tobacco and fragrance-free facility.

Required Qualifications

• Valid Physical Therapist license in Pennsylvania
• Minimum 2 years of experience as a Physical Therapist
• Physical Therapist experience within the last year
• Experience with Epic electronic health record system

Required Documentation & Compliance

• Government-issued photo ID.
• Background checks including 7-year criminal history, EPLS/GSA/SAM, HHS/OIG, and Pennsylvania-specific PATCH search.
• PA FBI fingerprinting required within 1 year of start; candidate may start with results pending but must have results on file within 45 days.
• Education verification of highest degree.
• Professional references (2 within last 3 years).
• Recent resume or employment application.
• Skills checklist (annual requirement).
• Physical exam within 90 days of start, signed by MD, DO, PA, or NPT.
• Immunizations and health requirements including:

  • TDAP vaccine within 10 years.
  • Negative TB test (QGold or TSpot) within 90 days.
  • Measles, Mumps, Rubella (MMR) vaccination or positive titers.
  • Hepatitis B vaccination series or positive titer (or declination).
  • Varicella vaccination or positive titer.
  • Seasonal flu vaccination or valid medical/religious exemption.
  • COVID-19 vaccine, medical/religious exemption, or declination.
• Drug screening (9-panel) within 1 year (for agency use only).
• Respiratory safety clearance within 1 year, renewed annually.
• Completion of facility-specific documents including medical statements, confidentiality agreements, code of conduct, and health & wellness commitments.
• Passport-style professional photo for badge.

Certifications (as applicable)

• ACLS (Advanced Cardiac Life Support)
• BLS (Basic Life Support)
• Other certifications as required by unit or specialty.

Additional Notes

• Candidate will be reviewed by clinical staff and manager before final approval.
• Must comply with facility policies including tobacco and fragrance-free environment.
• Floating may require travel across county lines.
• All health documentation must include two identifying markers (e.g., full name, DOB, or SSN).
