Job Opportunity: Physical Therapist - Home Health

Job Description

• Provide home health physical therapy services in various areas including Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, Los Gatos, and San Jose.
• Shift: 8-hour day shifts.
• Caseload: 5-6 patients per day (1:1 ratio; may visit up to 6 patients daily).
• Guaranteed 36 hours weekly (not applied during first 2 weeks, holiday weeks, or time off weeks).
• Weekends: Optional to pick up patients to meet productivity.
• On-call: Not required.
• Locals accepted.
• Own transportation required; occasional travel radius up to 50+ miles.
• Timely communication and documentation required; documentation tools provided.

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in a Home Health setting
• Current Basic Life Support (BLS) certification

• Attire: Any color scrubs.
• Parking: Free.
• Mileage reimbursement according to IRS standard.
• Compliance process takes about 2 weeks before start date.

Compliance & Onboarding Requirements (Summary)

• Government-issued photo ID.
• Background checks including 7-year history and annual updates.
• Certifications: BLS required; ACLS and PALS may be required depending on contract.
• Health documents including physical exam, immunizations (Tdap, MMR, Hepatitis B, Varicella), TB test, drug screening, flu vaccine or declination, and COVID-19 vaccine or declination form.
• Auto insurance and registration required.
• Emergency contact and other facility-specific forms must be completed prior to start.
• Professional references and work history verification required.

Notes for Candidates

• This position requires prior home health experience; first-time travel therapists will not be considered.
• Candidates must be prepared for a travel contract with a 13-week duration.
• Reliable transportation is essential due to travel between patient homes.
