Job Opportunity: Physical Therapist - Home Health

Location: San Jose, California
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

Specialty: Home Health Physical Therapy
Patient Load: 5-7 patients per day (primarily geriatric cases)
Guaranteed Hours: 40 hours per week
Territory: Up to a 45-mile radius from the office/hub
Dress Code: Scrubs (any color), no jeans
Equipment Provided: iPad
Mileage reimbursed at IRS standard rate
Cell phone use: Not provided/required
Flexibility with territory expected
Locals accepted

Required Qualifications

Valid Physical Therapist license in California
Minimum 1 year of recent Physical Therapy experience
Basic Life Support (BLS) certification required

Required Documentation & Compliance

Government-issued photo ID (Driver’s License or State ID)
Emergency Contact Form prior to start
Background checks within 1 year (including SS Trace, County searches, Nationwide, Sex Offender)
Annual EPLS/GSA/SAM and HHS/OIG checks
Certifications:
- BLS (Basic Life Support)
- ACLS and PALS preferred but not explicitly required for this role
Physical exam within 1 year of start
Negative TB test or chest X-ray within 1 year (with specific rules for past positives)
Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or history of chickenpox)
9-panel drug screen within 1 year of start
Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
Auto insurance documentation (personal or auto)
Professional references:
- 2 for travel, allied, or per diem acute care
- 1 for per diem non-acute care
Annual skills checklist and competency exams as applicable
