Speech and Language Pathologist (SLP) - Long Term Care (LTC)

Location: Moore, Oklahoma
Employment Type: Temporary

Contract Length: 13 weeks


Job Description

  • Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders.
  • Develop and adjust individualized plans of care using standardized assessments and evidence-based practices.
  • Assess patient progress and modify treatments accordingly, documenting all clinical and regulatory information in electronic records.
  • Evaluate treatment outcomes and make referrals as needed.
  • Provide consultation and counseling to patients, families, caregivers, and other service providers.
  • Communicate effectively with supervisors and healthcare team members regarding patient progress and treatment plans.
  • Prepare comprehensive discharge summaries per regulatory and clinical requirements.
  • Accurately record treatment minutes in compliance with all regulations.
  • Participate in patient care conferences, utilization review meetings, rehabilitation meetings, and required trainings or in-services.
  • Obtain necessary durable medical equipment (DME) or supplies as required.
  • Supervise and support Clinical Fellowship Year (CFY) or student Speech-Language Pathologists.
  • Adhere to ASHA and state practice acts, codes of conduct, and ethics.

Required Qualifications

  • Minimum 1 year of Speech-Language Pathology experience.
  • Experience in Long Term Care (LTC) or Rehabilitation settings.
  • Active Oklahoma State License.
  • Certification of Clinical Competence (CCC) from ASHA.
  • New graduates are eligible if they have completed Level II clinical experience (beyond Level I observation).
