Job Description

Aspirus At Home is seeking a Registered Nurse for home health services in Crystal Falls. The role involves providing care to patients in their homes, with a focus on quality and timely documentation.

Requirements

• Shift: 8-hour days, approximately 8 AM to 4:30 PM (flexibility may be available based on census).
• Hours: 40 hours per week.
• Travel Requirements:

  • 20-25 patients weekly.
  • 4-5 visits per day on average.
  • Daily travel can vary from 25-60 miles, with potential for over 100 miles in a large territory.
• Certifications Required:

  • AHA BLS
  • Active RN license in Michigan and Wisconsin
  • Valid Driver's License
  • OASIS certification is a plus.
• Experience:

  • Minimum of 2+ years in home health preferred.
  • Must have travel experience.
• Charting Experience:

  • OASIS required.
  • Epic required.

Additional Information

• Radius Rule: Must live outside 50 miles from the facility.
• Mileage Reimbursement: Available at the current government rate.
• Insurance Requirement: Minimum auto insurance coverage of $100,000/$300,000 or $300,000 combined single limit liability.
