Physical Therapist - Contract Opportunity
Location: Rutland, Vermont
Employment Type: Contract
Contract Length: 13 weeks
Job Description
- Monitor and adjust patient visit schedules to maximize productivity and meet patient needs.
- Perform comprehensive patient assessments including psychosocial, educational, safety, and risk factors in compliance with federal/state regulations and professional guidelines.
- Develop individualized plans of care to promote functional independence and safety, setting patient-specific goals and interventions.
- Provide skilled physical therapy treatments, including therapeutic techniques and patient/caregiver education, while assessing patient progress at each visit.
- Coordinate patient care with interdisciplinary team members to ensure optimal outcomes and communication.
- Supervise and direct clinical activities of PT assistants, students, and home care aides.
- Complete and manage required documentation and paperwork to reflect care provided and support billing processes accurately.
- Participate in training programs for healthcare staff, volunteers, and community groups.
- Maintain and enhance professional knowledge and promote physical therapy standards.
- Engage in marketing and promotion of rehabilitation services within the community.
- Perform patient-related health assessments (e.g., PT/INR, blood pressure) and procedures (e.g., staple/suture removal, wound care).
- Communicate schedule changes promptly to relevant staff.
- Complete OASIS documentation and collaborate with quality improvement staff to ensure accuracy.
- Follow agency protocols for visit documentation and billing compliance.
- Assess patient needs for home care aide services and coordinate plans of care accordingly.
- Coordinate safe patient discharges and transfers, ensuring all required paperwork is completed per policies and regulations.
- Report patient incidents in a timely manner.
- Participate in peer review audits and provide/accept professional feedback.
Required Qualifications
- Licensed Physical Therapist in the state of Vermont.
