Therapist MS – Local Contract

Job Description

• Provide comprehensive psychosocial assessments, coordination, planning, and implementation of treatment plans for patients in various departments such as Community Health, Palliative Care, and Cancer Center Community Program.
• Promote optimal mental health by offering ongoing psychosocial support to patients and families dealing with illness adjustment, grief, addiction, and other stressors.
• Collaborate with an interdisciplinary team to achieve treatment goals.
• Evaluate patients’ biopsychosocial needs using multiple factors.
• Develop treatment and discharge plans.
• Document assessments and interventions accurately and objectively.
• Maintain knowledge of community resources, programs, and social services.
• Educate patients and families on healthcare options and connect them with appropriate resources.
• May develop and facilitate support groups and workshops.
• May serve as a resource and preceptor to MSW and MFT interns.
• May assist with treatment schedules, patient assignments, and crisis interventions as requested.

Work Schedule

• Monday to Friday, 8:30 AM to 5:00 PM
• 8 hours per day, 40 hours per week

Required Qualifications

• Valid LMSW - Licensed Master Social Worker license in California
• Licensed Master Social Worker (LMSW) with at least 1 year of experience, including experience within the last year
• Bachelor's Degree or Master's Degree in Social Work (MSW)
• Experience in an Acute Care setting
• Social Worker experience
