Job Opportunity: Outpatient Physical Therapist
Location: Northampton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 17 weeks
Job Description
- Provide patient care for individuals requiring rehabilitation in an outpatient setting.
- Focus on optimizing patients' physical, functional, psychological, social, and emotional well-being.
- Schedule:
- Monday, Wednesday: 9:30 AM – 6:00 PM
- Tuesday, Thursday, Friday: 8:30 AM – 5:00 PM
- Outpatient orthopedic experience is required.
Required Qualifications
- Valid Physical Therapist license in Massachusetts
- Experience in an outpatient setting
- Outpatient orthopedic experience
Onboarding & Compliance Requirements
- Submission of a professional color photo (recent, plain background, no filters or eyeglasses).
- Verification of professional license and certifications (primary source verification required).
- Documentation of immunizations and health screenings including:
- MMR (measles, mumps, rubella) vaccination or immunity proof
- TB test within 3 months of start date (skin test or blood test)
- Health statement signed by a licensed provider within 1 year
- Criminal background checks including:
- Massachusetts CORI check
- National sex offender registry check
- Exclusion checks (OIG, GSA, Medicaid lists)
- Submission of resume covering past 7 years of work experience with city and state details.
- Two professional references within the last 4 years (at least one supervisor or higher).
- Completion of a skills checklist within 1 year of submission.
- Confirmation of assignment form upon acceptance and for any start date changes.
- Education verification through background check company.
- Social Security Number trace as part of background check.
Additional Preferred/Optional Health Requirements
- Hepatitis B vaccination or immunity proof (strongly preferred)
- Tdap vaccination within the last 10 years (preferred)
- If positive TB test, documentation of a negative chest x-ray is required.
