Job Description

Seeking a Physical Therapist for an outpatient setting. Shared housing is available (no pets or guests allowed). A car is required for this position. Current flu vaccine is mandatory.

Required Qualifications

• Valid Physical Therapist license in Massachusetts
• Minimum of 1 year of Physical Therapist experience
• Physical Therapist experience within the last year

Additional Requirements

• Criminal background checks including:

  • National and Massachusetts CORI (Criminal Offender Record Information)
  • Social Security Number trace
  • National sex offender registry check
  • Exclusion checks (OIG, GSA, Medicaid exclusion lists)
• Education verification of highest degree achieved
• Submission of a detailed resume including 7 years of work experience with city and state
• Two professional references within the last 4 years (minimum one supervisor-level)
• Completion of a skills checklist within 1 year of submission
• Confirmation of assignment form upon acceptance and for any extensions
• If moving into shared housing, a housing agreement is required
