Job Opportunity: Physical Therapist Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Summary

Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patient age range is from birth to over 100 years. Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services. Maintain thorough and timely electronic documentation and interdisciplinary communication in accordance with agency policies and standards of practice.

Requirements

Licensure & Experience

• Must hold a valid Physical Therapist professional license in Massachusetts.
• Minimum 1 year of Home Care experience required.
• Ability to travel to assigned cases.
• Weekend rotations occur once every 6 weeks.

• Specialty experience required: 1 year (Home Care Physical Therapy).

Credentials & Compliance Requirements

The following are required for onboarding (some required for submission):

• Professional Licenses & Verifications:

  • Massachusetts Physical Therapist license (primary source verification required).
  • MGB Badge Photo (recent professional photo).
  • Motor Vehicle Record (MVR) check within 45 days of start (required for all home care positions).
  • Criminal Background Check within 45 days prior to start, including Massachusetts CORI and National Sex Offender Check.
  • Education verification of highest degree achieved.
  • Resume including past 7 years of work experience.
  • Two recent references (minimum one supervisor or above).
  • Skills checklist specific to the role.
  • Confirmation of Assignment/POI form upon acceptance.
  • SSN Trace as part of background check.
  • Exclusion checks (OIG, GSA, Medicaid lists).

• Health & Immunization Requirements:

  • Fit Test for N95 respirator within 365 days of start, updated annually.
  • Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions allowed.
  • TB test documentation within 3 months of start (skin test or blood test), updated if >6 month break.
  • Health statement signed by MD/DO/NP/PA within 365 days of start, updated if >6 month break.
  • MMR vaccination or proof of immunity.
  • Hepatitis B vaccination strongly preferred (optional).
  • Tdap vaccine within 10 years preferred (optional).
  • Positive PPD requires negative chest x-ray documentation (optional).
  • Respirator Medical Clearance attestation (if fit test required).

Additional Information

• Weekend rotations are infrequent (once every 6 weeks).
• Ability to travel to patient homes as assigned is necessary.
• Documentation and communication are critical components of the role.
