Shift: Day shift (5 days per week, 8 hours per day; 8:00 AM – 4:30 PM)

Job Description

• Position in inpatient rehabilitation therapy.
• Extension candidates are not to be submitted.

Required Qualifications

• Valid Physical Therapist license in Kentucky

Health and Safety Requirements

• Annual physical exam or physician note within 1 year prior to start, stating clearance to work without restrictions.
• Fit mask test to be completed onsite; PAPRs not accommodated if facial hair present. Medical or religious exemptions must be submitted on day one.
• Immunizations and documentation required prior to start:

  • Tetanus/Diphtheria/Tdap (valid within 10 years; Td not accepted).
  • Negative TB test (QuantiFERON blood test within 1 year; T-Spot/PPD not accepted). If positive TB, chest X-ray within last 5 years plus TB questionnaire required.
  • Proof of 2 MMR vaccinations or positive titers for Rubeola (Measles), Mumps, and Rubella. If low/equivocal titers, proof of first dose to start and second dose within 28-30 days.
  • Hepatitis B series, titer, or declination (declination must be within 1 year of start).
  • Proof of 2 varicella vaccinations or positive titer with same dosing rules as MMR.
• Drug testing: Must be completed within 30 days of start using specified 9-panel bundle (no THC) with expanded opiates and oxycodone. Annual updates required.
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (9/1–3/31). Exemptions must be approved through the facility’s employee health system prior to start.

Facility-Specific Compliance

• Signed and dated specialty job description required annually.
• Completion of employee health attestation and instructions form prior to compliance deadline.
• Submission of screenshots proving completion of:

  • TB results upload
  • TB risk assessment questionnaire
  • OSHA respirator medical evaluation
  • Flu vaccine or exemption upload (during flu season only)
• Review and acknowledgment of fit test and badge instructions.
• Kentucky-specific vulnerable adult maltreatment registry verification required within 30 days prior to start and annually thereafter.
