Job Opportunity: Physical Therapist - Home Health

Location

• Pulaski, Tennessee
• Coverage area includes Lewis, Lawrence, Marshall, and Giles counties
• Must be willing to travel up to 75 miles from home clinic
• Travel approximately 30-40 miles per day

Employment Type

• Travel/Contract
• Contract length: 13 weeks
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hour shifts)
• Patient visits primarily in patients' homes
• No on-call required

Job Description

• Provide physical therapy services in a home health setting
• Average caseload: 5-10 patients per day (approximately 6-8 points per day based on visit types and activities)
• Types of cases: Start of Care and Follow-Ups
• Use EMR systems: HomeCare HomeBase, OASIS, Outlook
• No guaranteed hours; if patient census is low, may be floated to a nearby clinic to pick up hours
• Local candidates accepted at the same rate

Required Qualifications

• Valid Physical Therapist license in Tennessee
• Minimum 1 year of recent physical therapy experience within the last year
• Experience working with adult patients in a home health setting
• Current BLS certification
• Reliable transportation and current driver's license required
• Agency-required liability insurance required

Compliance and Documentation Requirements

• Must complete standard background checks including 7-year history, sex offender, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable
• Fingerprinting required based on state regulations
• Education verification for degree applicable to profession
• Drug screening: 10-panel drug test required
• Physical exam within 12 months confirming good health and no communicable diseases, updated annually
• Negative TB test or chest X-ray within 12 months, updated annually
• Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella as per standard requirements
• Seasonal flu vaccination or documented declination required
• COVID-19 vaccination series or declination required
• Two professional references required
• Verification of work history for at least the last 7 years or two most recent employers
• Must review and sign various compliance, confidentiality, privacy, and policy documents prior to start and annually if assignment extends beyond one year

Additional Notes

• Weekend requirements vary; discuss during interview
• Some positions may have alternate shift schedules (e.g., 4x10s Friday-Monday) but most are standard 5x8 Monday-Friday
• This is a true contract assignment with no option for hire at the end of the contract

This position is ideal for experienced physical therapists with recent home health experience who are comfortable traveling within a rural territory and managing a moderate patient caseload in a home setting.
