Case Manager (Registered Nurse) - Travel/Contract
Location: Oakland, California
Contract Length: 13 weeks
Job Summary
- Work a 5/8 schedule (40 hours/week), every other weekend off
- Day shift: 8:00 AM – 4:30 PM
- Responsible for care coordination and care transitions planning throughout the acute care patient experience
- Collaborate with physicians, utilization managers, social workers, bedside nurses, and interdisciplinary teams to ensure timely patient progression and appropriate level of care
- Focus on preventing unnecessary admissions or readmissions and facilitating communication across the care continuum
- May be assigned to Emergency Department to address complex clinical and social situations efficiently
Key Responsibilities
- Conduct initial and ongoing patient assessments, including medical, psychosocial, and financial information gathering
- Lead healthcare team in developing cost-effective care coordination plans
- Identify and report quality and risk management concerns
- Oversee Case Management Assistants for post-acute care service preferences
- Perform utilization management by reviewing medical records to confirm level of care (LOC) and expedite transitions
- Monitor length of stay and collaborate with financial counselors on inpatient stay denials
- Ensure compliance with Medicare Important Message delivery timelines
- Participate in patient rounds and interdisciplinary team collaboration
- Maintain documentation in electronic health records (EHR) and stay current with CMS, Joint Commission, and regulatory requirements
- Develop and communicate transition plans with patients, families, payers, and community resources
- Screen for 30-day readmissions and proactively identify barriers to care progression
- Address complex clinical and social issues to avoid improper care utilization and delays
- Ensure timely transitions to lower levels of care and follow-up appointment coordination
- Make appropriate referrals for psychosocial issues and financial counseling
- Participate actively in department operations and continuous improvement initiatives
- Maintain professional relationships and demonstrate cultural sensitivity
Required Qualifications
- Valid RN license in California
- Acute Care Case Management (5 years of experience)
- Acute Care Case Management experience within the last year
- BLS issued by American Heart Association or American Red Cross
- Associate of Science in Nursing / Associate Degree in Nursing
- Experience in an inpatient setting
Additional Information
- Must comply with all facility policies and procedures
- Participate in ongoing training and competency exams as required
- Demonstrate effective communication and teamwork skills
- Ability to manage complex patient care coordination in a fast-paced acute care environment
