Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and periodic office meetings.
- Average patients per day: 5-8.
- Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
- Must have reliable transportation, current driver's license, and agency-required liability insurance.
- Must be willing to travel up to 75 miles from the home clinic.
- Local candidates accepted at the same rate.
- Some positions may follow a Baylor Shift schedule (4x10s, Friday-Monday), but most are standard 5x8, Monday-Friday.
Requirements
- Minimum 1 full year of recent Adult Home Health or Hospice experience (depending on specialty).
- First-timers are not accepted; recent specialty experience is required.
- CPR and BLS certification required.
- Must have an active license in hand; pending licenses are not accepted.
- Proficiency with HomeCare HomeBase (HCHB) charting system within the last 6 months is required. Retraining may be required if last use was over 6 months ago.
- Weekend and on-call requirements vary by location and must be discussed during the interview.
Work Environment
- Dress code: Scrubs.
- Documentation and charting primarily done through HCHB.
- Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).
Compliance and Documentation Requirements
Candidates must complete and maintain the following (as applicable):
Certifications
- Current BLS (Basic Life Support) certification from AHA, National CPR Foundation, or ARC.
Background Checks & Authorizations
- Standard background check covering the last 7 years of residence, work, and education.
- Sex offender search.
- EPLS/GSA/SAM and HHS/OIG checks.
- Fingerprinting as required by state (varies by location).
- Education verification.
- State-specific background and registry checks depending on assignment location.
Health Documents
- Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- Drug screening (10-panel).
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or documented declination.
Facility-Specific Documents & Training
- Personal or auto insurance (policy must be current and under candidate’s name or list candidate as a driver).
- Review and sign various compliance documents including:
- Code of Conduct
- Confidentiality forms
- HIPAA and Privacy Policy
- Compliance training and quizzes
- Home Health or Hospice core policy acknowledgments and scenarios (as applicable)
- COVID-19 policy and attestation
- Other state or facility-specific forms as required
Work History & References
- Verification of at least two most recent employers or last 7 years of work history.
- Two professional references required.
This position requires a dedicated LVN/LPN with recent home health or hospice experience, strong clinical skills, and the ability to work independently in a home care setting. Candidates must be prepared to meet all compliance, health, and documentation requirements prior to start.
